Rhinoplasty

Rhinoplasty

The nose is the most attractive organ that gives a three-dimensional appearance to the face. It also has vital functions and provides an aesthetic appearance. The smoothness and functions of the internal and external structures of the nose are very important for our breathing easily.
When the shape of the nose is inconsistent with the structure of the face or makes breathing difficult, here aesthetic nose operations are the most appropriate solution, the motivation for performing rhinoplasty may be to change the shape of the nose, improve breathing, or both.
Rhinoplasty must be well planned and applied meticulously to achieve improvement in both aesthetic and functional aspects

Who is the candidate for the operation?

When the shape of the nose is inconsistent with the structure of the face, has deformity after trauma, or makes breathing difficult, he is a candidate for rhinoplasty
Here it is performed to change the shape of the nose or to eliminate the factors that make breathing difficult, rhinoplasty is performed by surgeons with experience in the branches of the ear, nose, and throat and plastic surgery, which are frequently performed in the world and are considered a low-risk surgeries, Turkey is at the top of the list of countries Most likely to do rhinoplasty
The important thing here is that the person shares the parts he wants to correct with his doctor, and the doctor must determine what the patient can change, the doctor will start the process with good planning after comparing the structure of the nose, cartilage, face shape, and skin type given the patient’s desires.

Preparation before rhinoplasty

A detailed interview is done before the operation. After the examination, information about the inside of the nose is obtained by means of a CT scan when necessary. The doctor will also be informed of medicines unsuitable for use before and after surgery

Duration of surgery and anesthesia

In general, 2-2.5 hours is enough. Rhinoplasty can be performed under general anesthesia or local anesthesia, however, it is much preferable for patients and surgeons to operate patients under general anesthesia.

Surgery technique

If there is minimal nasal deformity (for example, a very small arc), closed surgery may be sufficient. However, an open technique is used in the surgical procedures, which will generally change the general appearance of the nose, which also requires procedures for the tip of the nose. In this technique, the operation is performed by separating the skin of the nose, cartilage and bone under it, with an incision called columella and is performed from the structure in the middle of the gills.
When the arch on the back of the nose is straightened, the roof of the nose becomes open, and it is necessary to remove this opening by breaking the nasal bones. Additionally, the nasal bones must be fractured and corrected in order to restore the bony structures that have healed abnormally after nasal base shrinkage or trauma.
Surgical suturing materials and cartilage grafts are needed in the cartilage tissues outside the nasal bones, especially in forming the tip of the nose. Cartilage tissue taken from the nasal septum for first-time patients, which we usually call primary rhinoplasty cases is sufficient.
In cases where excessive cartilage tissue is required, cartilage tissue taken from the ear or rib may be used.
At the end of the surgery, silicone pads are placed inside the nose and a thermoplastic splint is placed outside the nose, which is taken 2 and 7 days later, respectively.

Postoperative period

Although there may be nausea and dizziness in the early period after rhinoplasty, these conditions can be overcome with medication. Although post-operative pain is expected, which may vary with a person's pain threshold level, the presence of effective pain relievers helps to easily cope with this annoying condition.
There may be swelling of the face and bruising around the eyes. The source of bruising is usually the blood that seeps under the skin during bone formation and does not cause any problems. Ice therapy that will be applied in the early postoperative period is very effective in reducing bruising and pain. Temporary loss of feeling, fullness, and swelling in the nose may appear after surgery and these symptoms will improve over time
Irrigation with saline or similar fluids is useful for preventing crusting in the nose, closing the respiratory tract, and moisturizing the mucous membrane after surgery.

Possible complications of surgery

As with any surgery, rhinoplasty can have complications. These include infection, respiratory distress, inability to obtain the desired aesthetic gains from surgery, permanent numbness, septal perforation, and nasal deformity.
In such cases, it is generally possible to overcome the problem by performing revision surgeries, but it is important to prevent complications by taking the necessary precautions before and after the operation. In studies conducted worldwide, it was determined that 5-10% of surgical interventions require revision
